Step-by-Step Guide
  1. 1Sit upright in a comfortable position with your shoulders relaxed to allow full diaphragmatic excursion.
  2. 2Place the IMT device (or a restrictive breathing tool) in your mouth, ensuring a tight seal with your lips.
  3. 3Exhale fully and naturally to empty your lungs.
  4. 4Inhale forcefully and deeply through the device against the resistance, expanding your lower ribcage and abdomen.
  5. 5Exhale slowly and passively without resistance.
  6. 6Repeat for 30 breaths or the duration prescribed by your clinician, resting if you feel dizzy.
Physiological Mechanisms

IMT operates on the principle of progressive overload applied to the respiratory muscles, primarily the diaphragm and external intercostals. By inhaling against a mechanical resistance (often using a specialized threshold device), it induces muscle hypertrophy, increases the proportion of Type I (endurance) muscle fibers, and enhances neuromuscular recruitment. This leads to improved ventilatory efficiency, delayed onset of respiratory muscle fatigue, and reduced perception of dyspnea during physical exertion.
